For other medicals, please ensure that you bring with you a copy of the medical form that you need us to complete AND a brief summary of your medical record from your NHS GP, to include a list of your current and past medical problems and your current and past medication. We need this because otherwise we do not have access to your NHS record, and most medicals require that we confirm the absence of certain conditions with reference to the record. Most NHS GP practices are willing to print off this brief summary in reception as you wait.
Women’s Health and Wellness
There are many different ways to maintain a healthy life. We offer a bespoke service to support this journey and address any concerns.
The 45-minute appointment will include:
- Detailed history and examination
- Explore any particular health concerns
- Identify any woman-specific health screening requirements
- Osteoporosis assessment
- Comprehensive laboratory blood testing, including cholesterol, diabetes check, bone health profile, thyroid function, iron studies and Bowel Cancer screening kit
Following your medical, we will send you a copy of your blood test results and, if necessary, personalised recommendations and interventions.

If Bone density X-rays (DEXA scan), Pelvic Ultrasound, Ca125 blood test, hormone profile, Mammogram, or smear tests are required, these can be arranged, and there will be an additional charge.
Men’s Health and Wellness
Our Well Man Health Check is designed to give men a clearer picture of their health and to help identify and address potential health issues before they become more serious.
The 45-minute appointment will include:
- Detailed history and examination
- Explore any particular health concerns
- Identify risk for any male-specific health conditions
- Comprehensive laboratory blood testing, including cholesterol, diabetes check, prostate profile and Bowel Cancer screening kit

Prostate Health Review
A shorter, focused appointment for men who want to assess prostate cancer risk, have a family history of prostate cancer or are simply keen to have proactive prostate screening, or address urinary symptoms.
This medical aims to address these specific issues.
The 30-minute appointment will include:
- Detailed history and examination as appropriate
- Explore any concerns about urinary tract symptoms
- Assess individual risk of prostate cancer
- Laboratory testing for prostate function (total and free PSA), kidney function and full blood count
Following your medical, we will send you a copy of your blood test results and, if necessary, personalised recommendations and interventions.
Price: £185
If a Prostate MRI is required, an additional charge will apply.
We can also now offer the Stockholm 3 blood test, an evidence-based test that can help stratify risk in those aged 45-75 with a PSA between 1.5 and 20, costing £395.
Cancer Screening
Screening for cancer means detecting cancer at an early stage (I or II) before it presents itself. Early detection increases the chances of effective treatment and survival.
Your individual risk for cancer has many variables, including your genetics and lifestyle. We can offer comprehensive investigations, including examinations, blood tests, and radiological tests (such as ultrasound and CT scans).
Working with our clinical partners at Myogenes and TDL London, we can arrange appropriate genetics testing or screening blood tests.

Firearms Medical – for registered patients
Please arrange a 15 minute appointment. This must be booked with Dr Charlie Middle, either in person or online (prior to your appointment, please send the completed papers to the practice).

Firearms Medical – for new patients
Please fill in a New Patient Registration form and then arrange a 30 minute F2F appointment. This must be booked with Dr Charlie Middle.
You must bring your full medical notes to your appointment which can be accessed from your current GP.

An ML5 is a domestic medical certificate that seafarers or fishermen need to get to certify they are medically fit to work safely at sea.

A DVLA Group 2 (D4) medical assessment is required for professional and vocational drivers, including HGV, bus, taxi, and motorsport licence holders, to confirm fitness to drive safely.

Why have a medical if I feel well?
Many important health conditions develop without symptoms. A medical can help identify risk factors such as high blood pressure, raised cholesterol, diabetes, cardiovascular disease and cancer before they cause problems.
A health assessment also provides a useful baseline for monitoring your health in the future.
Do you offer bespoke health assessments?
Yes. Unlike many standard screening packages, our approach is personalised. We take into account your medical history, family history, lifestyle, age, health concerns and future goals before recommending any investigations.
We believe that understanding the individual is just as important as the tests themselves.
Do you provide HGV and taxi medicals?
Yes. We provide DVLA Group 2 medicals for HGV, LGV, bus and coach drivers, as well as taxi driver medicals required by many local authorities.
Please bring any forms or documentation required by your licensing authority.
Do you provide firearms and shotgun certificate medicals?
Yes. Dr Charlie Middle provides medical assessments for shotgun and firearms certificate applications and renewals and can complete the relevant documentation if appropriate.
Do you provide visa medicals?
Yes. We can advise on medical examinations required for visa applications, emigration, overseas employment and residency applications.

Do you provide motorsport medicals?
Yes. We undertake medical assessments for drivers and competitors participating in motorsport events and competitions. Please bring any paperwork supplied by your motorsport governing body.
Do you provide occupational medicals?
We can offer selected occupational and employment medicals, including pre-employment assessments and fitness-for-work medicals. Some of these are highly specialised and can only be provided by specially trained occupational health clinicians.
